Clean and thoroughly dry the damaged area.
Cut the patch to the desired size.
Remove the protective film and place the patch on the area to be repaired.
Fix it with an iron (medium heat, no steam) for about 30 seconds.

Yes, after fixing, the garment can be washed normally (up to 40 °C). The patch remains permanently attached. Use of a dryer is not recommended.
